First Visit
What Will Happen On My First Visit?
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. We will complete an in-depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease.
Please assist us at the time of your initial visit to the office by providing the following information (if you have it). If you do not have any of the following information, do not worry. We will assist you in getting it. (Please note that you can fill out your patient registration and medical history form here.)
- Your referral slip (if applicable)

X-rays – If your dentist or physician has taken x-rays, you may request that they forward them to our office, preferably via e-mail to [email protected]. You may also pick them up from your dentist and bring them to our office. If additional films are necessary, they can be taken at our facility.
- A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website.)
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension) or if you are taking any medication (e.g. heart medications, aspirin, anticoagulant therapy etc.)
- Your Dental Insurance Information (If you have dental insurance, please bring any forms or insurance cards with you to the appointment. This will save time and allow us to help you process any claims.)
Please note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
